New Patient Registration: Click below to begin the enrollment, add details, agree to the Terms of Service (i.e. Patient agreement, HIPPA policy), place a credit card on file, and wait to be confirmed as a patient. Parents: don't forget to make yourself your child's emergency contact. After confirmation, you will receive an email with additional forms to complete and we will call you to schedule an appointment. You will only need to enroll once.

Pricing and Membership Information
Initial Wellness Visit to Establish Care (and all other subsequent wellness visits): $150
Sick and Office Visits (which includes Televisits; In House Labs are an additional charge): $135
Discounts given for multiple children in a family.
Consultations for Chronic Medical Conditions: To be discussed with physician
Integrative Pediatric Membership: Designed for families of children with chronic medical conditions who want an integrative approach to getting to the root cause of the problem. Each membership is patient specific and tailored to meet specific needs; it can be a 3, 6, or 9 month treatment plan which includes office visits, ordering of labs and imaging, and a tailored treatment approach.
